How to Apply for the HCV program through the Region XII Regional Housing Authority

There are three ways to get an application:

  1. Pick up an application from our office in Carroll, IA.

  2. Meet with one of our representatives when they are in your area. Our staff goes to several counties in our jurisdiction monthly to accept applications, help individuals with completing additional paperwork, etc. Call our office to make an appointment.   

  3. Call our office to request an application to be mailed to you. You will still be required to return the application in person to our office or by making an appointment in one of our counties we visit monthly. 

 

ALL APPLICATIONS MUST BE RETURNED IN PERSON! If you mail, fax or email the application, you will be notified that your application has not been accepted due to improper submission and will need to resubmit the application.

 

Only complete applications will be placed on the Waiting List.  A complete application requires the following:

  1. Provide BIRTH CERTIFICATES and SOCIAL SECURITY CARDS for ALL household members.

   2. Provide government issued photo IDs for all adult household members (18+ years old).

   3. Complete all paperwork (front and back) in its entirety, including signatures for all adult household members.

   4. There are more required forms for all adults to sign when you return the application in person. All adults must be present when the application is returned.

If you have a specific circumstance that prevents you from providing all the above, please contact our office directly to discuss other options to complete an application.

If your application is not complete, please understand that our staff will send all the paperwork home with you to be completed and returned all together. We will not hold onto incomplete applications.

A complete application receives a date and time stamp and is added to our Waiting List.  Applications are processed in the order received. Please be patient and wait until your application is pulled from the waiting list and you are contacted.

When your application comes off the waiting list, we will perform a criminal/sex offender background check and contact you to update your financial situation and request additional documentation to determine final eligibility for the Housing Choice Voucher program. If your phone number or mailing address changes at any time, please contact us to update your information.

You will receive notification if you are eligible or ineligible for the Housing Choice Voucher program. If you are eligible, all adults must attend a Briefing Meeting at our office in Carroll, IA to learn more about the program, rules and regulations, and what you do next to find suitable housing to meet your family’s needs.
